Use this form to request an appointment with us. While we will do our best to accommodate your requested day and time, please note, your appointment is not fully booked until you get a confirmation from us!

PLEASE READ BEFORE SCHEDULING APPOINTMENT. 

We are not in network with any HMO or state plans.
We do not accept third party auto cases. This means that all auto claims must be processed though a patients personal injury protection policy.
After scheduling, we will send you an email with information about our clinic and a link to our New Patient Paperwork. If this paperwork is not completed within 24 hours of your appointment time you may be asked to reschedule. 

Please note, not all providers are in network with the same insurance companies. Please contact the office for further information. 

*Dr. Lauren will be out of the office on maternity from approximately July-October 2023. She will not be seeing NEW PATIENTS as of June 2023. If you are scheduled with her, your appointment may be rescheduled with Dr. Alex. 

By scheduling an appointment online, you will be agreeing to the following:
Our office will be reaching out to you within 48 hour after scheduling appointment to collect additional information. If we do not hear back from you within 48 hours than your appointment will be cancelled.
If you cancel your appointment within 48 hours of your scheduled appointment time, it will result in a No Show/Late Cancelation Fee of $150. Please help us serve you better by keeping scheduled appointments.
