We are not in a network with any HMOs or state plans.

We do not accept third-party auto cases. This means that all auto claims must be processed through a patient's personal injury protection policy.

After scheduling, we will email you with information about our clinic and a link to our new patient paperwork. If this paperwork is not completed within 24 hours of your appointment time, you may be asked to reschedule.

Please note that not all providers are in a network with the same insurance companies. Please contact the office for further information.

By Scheduling an Appointment Online, You Will Be Agreeing to the Following:

Our office will reach out to you within 48 hours after scheduling an appointment to collect additional information; if we do not hear back from you within 48 hours, then your appointment will be canceled.

If you cancel your appointment within 48 hours of your scheduled appointment time, it will result in a no-show or late cancellation fee of $150. Please help us serve you better by keeping your scheduled appointments.
